New Patient Registration

We accept patients living within a wide geographical area extending to the A38 in the west, Ipplepen to the north, Moreleigh to the south and the Torbay by-pass to the east.

Please take a look at our letter How to Register and download our New Patient Questionnaire.

We operate an open list system at Catherine House Surgery and welcome requests for registrations from patients living in and moving to the Totnes area.

In order to register we ask kindly that you complete two forms:

Family Doctor Services Registration (GMS1) Form 

– purple in colour, please ensure that you sign this.

New Patient Questionnaire (or Young Patient Questionnaire – if you are aged under 16)

In addition, if you do not wishto participate in the NHS Summary Care Record scheme then please complete the Summary Care Record Opt-Out Form.

All these forms are available from our reception or alternatively can be downloaded from the New Patient Registration section of our website to be printed, completed and brought into us.

Please bring some photographic ID and proof of address with you.  If you have any difficulties providing this information then please let us know.  Children under the age of 18 need not supply evidence and can be registered by the parent/guardian. 

You are invited to attend an initial appointment at the surgery with either one of the doctors or nursing staff.  Please feel free to make an appointment at your convenience.

Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as a recent utility bill.

You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team.

Medical treatment is available from the date of registration.  Please contact reception for further information.
